UWS Staff Gym Membership Application


This form has three sections leading to you becoming ready to use the UWS campus gyms.

You must read and acknowledge your understanding of all key documents listed below relating to the use of gym facilities, then you must complete the membership application process and agree to our fitness declaration.

The membership agreement includes unlimited use of UWS operated gyms and unlimited use of our fitness classes. Access restrictions will apply on our Dumfries campus due to College teaching delivery.

Any information submitted will be used solely for the administration of your gym membership.

This application form is for UWS STAFF members only.

I declare that to the best of my knowledge, the information given is correct and I know of no reason why I should not participate in an exercise programme or fitness class. I understand that I enter into any exercise programme entirely at my own risk.

Please Note: If you are aware of any medical conditions that may affect you whilst participating in an exercise programme, or your health status changes in the future, we recommend you consult with your doctor before continuing your exercise programme.

First aid trained staff members are available within the UWS operated gyms and wider University. If you feel unwell, please don’t hesitate to speak to a member of the gym team.

Please be advised that all our UWS operated gyms will have staff available during operations should you need any further guidance whilst visiting.
